For the convenience of our patients, we have taken many steps to lighten the burden of insurance filing. Our computer system enables us to do electronic filing directly with the insurance companies. And, we participate in a multitude of PPO programs...along with most regular major medical insurance companies.

Since policies vary from employer to employer, even within a given group or company, it is necessary to confirm the details of your coverage prior to beginning treatment. We have provided a print-friendly form to help you gather your specific information. Please do not assume you know all of the answers to the questions on the form. Unlike traditional medical policy applications, chiropractic often has varying levels of coverage and unique limitations.
